The agent wrote a help doc explaining the permission UI:

  The dialog begins with "Do you want to <action>?" followed by the menu.

This prose describes a menu but has NO live ❯ arrow at the start of any
numbered line. The permission detector requires BOTH the "Do you want to"
phrase AND "^\s*❯\s*[0-9]+\.\s*(Yes|No|Allow|Deny)" to trigger. Must stay
active/idle, not fire permission state.
